Pyruvaatkinase (PK) is een sleutelenzym in de glucosestofwisseling en komt voor in verschillende isovormen die weefselspecifiek zijn. In de actieve vorm bestaan al deze isovormen uit vier gelijke onderdelen (tetrameren). Bij de ontwikkeling van tumoren treedt een verlies op van het weefselspecifieke iso-enzym in de tetramere vorm.
Bij een hoge celdelingsactiviteit in het tumorweefsel, stemmen de cellen namelijk af op een metabolische “inkorting” om zo energie te sparen. Het aandeel iso-enzym type M2 neemt dan toe. Deze bestaat niet uit vier, maar slechts uit twee gelijke delen (dimere vorm) en bezit slechts weinig activiteit. Het dimere enzym M2-Pk wordt altijd door tumoren afgegeven en wordt daarom ook wel ‘tumor M2-Pk’ genoemd.
Deze parameter dient om zowel bloedende als niet-bloedende carcinomen te ontdekken. Door de meting van tumor M2-PK in feces worden met name colorectale tumoren opgespoord.
