HPU, hemopyrrollactamuria (also known as KPU), is a metabolic problem/condition that occurs more frequently in women than men. HPU does not always immediately lead to symptoms, but many people eventually experience health problems. A characteristic and direct symptom of HPU is increased joint mobility (hypermobility). Especially in women, it is common to be able to bend their little finger straight back or hyperextend their thumb. Hypermobility of the jaw joint is also common.
The abbreviation HPU stands for hemopyrrollactamuria and is named after the substance excreted in the urine of people with HPU, the so-called HPL complex (Hemopyrrollactam complex). This complex chemical compound is not found elsewhere in the body and is – it is suspected – a waste product of metabolism. This excretion of HPL is accompanied by a loss of important vitamins and minerals, particularly vitamin B6, zinc, and manganese.
Because HPU binds the active form of B6 (pyridoxal-5-phosphate), it disrupts the biochemical processes in the body where this vitamin is necessary. This primarily concerns the absorption of zinc, manganese, chromium, and magnesium from our food and the metabolism of carbohydrates and fats. Zinc and manganese are essential for resistance to infections, sugar metabolism, and cartilage formation. Vitamin B6 is also involved in the production of vitamin B3, which is why this vitamin is also reduced in HPU.
HPU should not be considered an independent disease and does not develop into a specific disease, but rather into a range of (mild to severe) symptoms that can lead to certain other conditions. HPU is stress-related, both psychologically and physically.
Insufficient evidence in mainstream medicine
The underlying condition or the clinical significance of this test is insufficiently supported within mainstream Dutch medicine.
Rationale for this test
Het ziektebeeld HPU/KPU wordt binnen de Nederlandse reguliere geneeskunde niet erkend. In 2006 oordeelde de Rechtbank Den Haag dat HPU geen wetenschappelijk onderbouwde aandoening is. De test meet een stof (hemopyrrollactam) waarvan de klinische betekenis niet is aangetoond in gecontroleerd wetenschappelijk onderzoek. HPU/KPU komt niet voor in Nederlandse zorgrichtlijnen (NHG, FMS) of internationale classificatiesystemen zoals ICD-10.
Do not interpret a result from this test as a diagnosis, and always discuss with your GP which regular diagnostics are appropriate for your symptoms.

After ordering we ship the collection kit to your home. You collect urine following the instructions, return it in the envelope, and receive your result digitally by email.
How do I collect my urine sample correctly?
Follow the included instructions, usually the midstream method in a sterile container. Pay attention to the right timing (first morning urine or random, depending on the test).
Do I need to avoid anything before collection?
For some tests yes (e.g. certain foods or supplements). Whether this applies is stated in the instructions. Read them before you start.
When and how do I receive my result?
Usually 2 to 5 working days after the lab receives your sample. You get an email with a link to your digital result, including explanation per value.
How do I return my sample to the lab?
In the included return envelope with free medical shipping. Drop it in the mailbox; the lab processes your sample as soon as it arrives. For some tests it's important to send it the same day — the instructions make this clear.
What if I have questions or something doesn't work?
Feel free to contact us via WhatsApp, email or phone. We're happy to help you before, during or after your test. If the collection doesn't work, we'll arrange a solution or send new materials — no hassle.
