About PSA testing: screening vs. indication
Free Prostate-Specific Antigen (Free PSA) is a protein produced exclusively by the prostate gland. It is a form of PSA that is not bound to other proteins in the blood. Measuring the level of free PSA helps in assessing prostate health and can be used in combination with the total PSA test to better estimate prostate health.
Testing Free PSA is particularly useful for men who have an elevated total PSA value. It helps in distinguishing between different prostate conditions, such as prostate enlargement or inflammation, which can also cause an elevation in PSA levels. By measuring the free PSA level, the necessity of further invasive examinations, such as a prostate biopsy, can be better assessed.
A ratio of free PSA to total PSA can help in assessing prostate health. A lower ratio generally indicates a higher risk of prostate issues, while a higher ratio may point more toward benign (non-serious) prostate conditions.
A high level of free PSA relative to total PSA may indicate a lower risk of prostate issues. It possibly indicates the presence of benign prostate conditions.
A low level of free PSA relative to total PSA may be an indication of a higher risk of prostate issues. This may be a sign that further investigation is necessary to assess prostate health.
This test has clinical value for specific complaints or indications, but is not intended as screening without symptoms.
Rationale for this test
Vrij PSA wordt in de Nederlandse prostaatcarcinoom-richtlijn genoemd als aanvullende test bij een negatieve totale PSA-uitkomst, maar is niet geschikt als standaard-screening zonder indicatie. De test heeft klinische waarde bij het beoordelen van mannen met een verhoogde totale PSA-waarde, waarbij de verhouding vrij/totaal PSA helpt bij het onderscheiden van benigne prostaataandoeningen en het inschatten van de noodzaak voor verdere invasieve diagnostiek zoals biopsie.

At most collection points yes. Some accept walk-ins. This is clearly indicated per location. You choose the location that works best for you.
