Bij deze test worden bij een vrouw de hormonen oestradiol, progesteron, testosteron en DHEA gemeten in speeksel. Niet alleen voor de bijnierschorshormonen cortisol en DHEA, maar ook voor de bepaling van de geslachtshormonen oestradiol, progesteron en testosteron is de concentratiebepaling in speeksel zinvol.
In gebonden vorm zijn hormonen inactief. Voor de vaststelling van specifieke hormonale activiteiten is het daarom belangrijk om de concentratie vrije hormonen te bepalen. Ongeveer 95 tot 99% van de bijnierschorshormonen in bloed is aan eiwitten gebonden (bijvoorbeeld CBG (corticosteroïd-bindend globuline), SHBG (geslachtshormoon-bindend globuline) en albumine). In speeksel daarentegen is dit slechts 1-5%.
