The human colon contains 10¹¹ bacteria per gram, making it the largest bacterial population in the body. Most of these are anaerobic.
The gut microbiota consists of a resident and a transient gut microbiota. These consist of aerobic, anaerobic, and microaerophilic gut bacteria.
The transient gut microbiota, also called passing gut microbiota, is ingested through food. It consists of bacteria that are tolerated to some extent, but they are generally not beneficial and some are pathogenic.

An important part of mapping the aerobic transient gut microbiota is the examination of Enterobacteria. Here, the qualitative determination of the various Enterobacteriaceae species is central. Among the Enterobacteriaceae species, there are a number of species that have various pathogenic properties. Some obligate pathogenic species from this family are Salmonella, Shigella, and Yersinia species. Urease-producing species like Proteus species can increase serum ammonia concentration in patients with impaired liver function. Various species of Enterobacteriaceae are involved in rheumatoid reactions, including Enterobacter and Klebsiella species.
Most Clostridium species grow under anaerobic conditions. An amount of < 104 is acceptable for the intestinal environment. Due to their metabolic activity, using carbohydrates, proteins, and fats, they can cause burdens if the tolerance limit is exceeded. Especially the ability to split fats can be the cause of harmful substances. Often, an increased presence of Clostridia is associated with a decreased amount of Bifidobacteria and/or Bacteroides.
Clostridium difficile is a gram-positive, spore-forming bacterium and the main cause of diarrhea and colitis associated with antibiotics. The mortality in older people infected with this bacterium is approximately 25%. The bacterium is found in 5% of adults and 15 to 70% of children. The majority of infected hospital patients are asymptomatic carriers of the bacterium.
Another common species is Clostridium perfringens. C. perfringens is a gram-positive, anaerobic, spore-forming, rod-shaped bacterium and causes necrotizing and histolytic toxins (lecithinases) and enzymes such as proteases and collagenases. Clostridium perfringens triggers inflammatory processes. Infections with C. perfringens often occur due to the consumption of poorly chilled meat.
Bacteria from the Enterobacteriaceae group 1 and 2 primarily metabolize proteins. They are more common in large-scale inflammations. They are burdensome for the liver due to the production of toxins, ammonia, and biogenic amines.
The use of antibiotics increases the risk of a salmonella infection. These infections are more common in young children. The most common species are S. typhimurium, S. heidelberg, and S. enteritidis. Sources of salmonella infection include: contact with sick individuals, living in rural areas, recent (foreign) travel, contact with infected pets, and consumption of unpasteurized dairy products and undercooked meat. Shigella species are also often responsible for intestinal infections causing diarrhea.
The underlying condition or the clinical significance of this test is insufficiently supported within mainstream Dutch medicine.
Rationale for this test
Het concept 'transiënt darmbioom' als diagnostische categorie wordt binnen de Nederlandse reguliere geneeskunde niet onderbouwd. Hoewel individuele pathogene bacteriën (zoals Salmonella, Shigella, Clostridium difficile) wel klinisch relevant zijn, bestaat er geen gevalideerde test die 'passerende darmflora' als geheel in kaart brengt met bewezen klinische betekenis. De onderverdeling in 'residente' versus 'transiënte' darmbiota is geen standaard-diagnostisch kader in NHG- of FMS-richtlijnen voor darmklachten.
Do not interpret a result from this test as a diagnosis, and always discuss with your GP which regular diagnostics are appropriate for your symptoms.

Practical information
After ordering we ship the kit to your home. You collect a small sample with the included scoop, keep it cool and return it in the envelope. You receive the result digitally by email.
Lay plastic film over the toilet or use a clean container. Scoop from the middle of the stool and avoid contact with urine or toilet water.
A small scoop with the included spoon is enough — about half a teaspoon. More is not better.
Maximum 24 to 48 hours in the fridge. Send as soon as possible and preferably early in the week, not just before the weekend.
Some analyses (like calprotectin) ask you to avoid NSAIDs or certain supplements 24-48 hours before collection. Check the instructions with your kit for the exact preparation for your test.
Stool analyses typically take about a week after the lab receives your sample. You receive your laboratory report securely by email. It lists the measured values, units, reference ranges and any flags added by the laboratory.
Use the supplied return envelope and place it in the letter-mail bag inside a PostNL service point yourself as soon as possible. Do not hand it to counter staff or request track & trace. The instructions supplied with your test state when it must be sent.
