What is Anaplasma phagocytophilum IgG and IgM?

Anaplasma phagocytophilum is a bacterium that grows in white blood cells and can reach people through a tick bite. The disease is called anaplasmosis and usually causes flu-like symptoms with fever, headache and muscle pain, which often go away after a few days. This test measures two types of antibodies against the bacterium: IgM and IgG.

Why test for Anaplasma IgG and IgM?

With fever, headache or muscle pain in the weeks after a tick bite, if you want to know whether Anaplasma may be the cause. Anaplasmosis is rare in the Netherlands: the bacterium is found in 2 to 3 out of 100 ticks, but the disease is only diagnosed very occasionally. As a check after a tick bite without symptoms, the test is not useful.

In the first week of symptoms, the antibodies often cannot yet be detected. The result says most if the blood is drawn a few weeks after the symptoms start, and even more with a second test a few weeks later. With high or persistent fever after a tick bite, a doctor should see you; do not wait for this result.

What does it indicate?

The test shows whether your immune system has had contact with Anaplasma or a related bacterium. One result does not say when that was: IgG can remain detectable for months to years, and for this bacterium IgM is not a reliable sign of a recent infection.

What does a positive result mean?

A positive IgG means that you have had contact with Anaplasma or a related bacterium, such as Ehrlichia, at some point. A positive IgM may fit a recent infection, but is often wrong with this test. A recent infection only becomes clear from a clear rise in IgG in a second blood draw 2 to 10 weeks later, or from a PCR on blood in the first week of symptoms. Anaplasmosis is not notifiable in the Netherlands.

What does a negative result mean?

If IgG and IgM are both negative, a past infection with Anaplasma is less likely. Shortly after the symptoms start, the antibodies may still be absent, so a negative result in the first week does not rule out anaplasmosis.

Preparation and sampling

  • Preferably do not have your blood taken in the first week of symptoms; the antibodies are often not yet detectable then.Applies to: Anaplasma phagocytophilum IgG and Anaplasma phagocytophilum IgM

Risks and downsides of this test

  • Blood draw at a collection point. A needle in a vein is a safe routine procedure. A bruise or slight dizziness happens; an infection of the puncture site is rare.
  • No test is one hundred percent reliable. A result can point to something that is not there, or miss something that is. A result is a snapshot and does not make a diagnosis.
  • An abnormal result has consequences. It can lead to follow-up tests with your GP, and to worry that turns out to be unnecessary. Follow-up tests or treatment can have downsides of their own.
  • Incidental findings are not expected with this test. An incidental finding is something other than what you were looking for. During checkout you choose whether you want to be informed about one.

Frequently asked questions

How does it work from order to result?

After ordering we generate a referral letter and email you the instructions. You pick a collection point yourself and book an appointment there. Blood is drawn at the location, processed at the lab, and your result arrives digitally.

How quickly can I get tested?

Often within a few working days. How soon you can be seen depends on how busy the location is and the type of analysis. For specialist analyses we first ship collection materials, which takes a bit longer.

Do I need to book an appointment?

At most collection points yes. Some accept walk-ins. This is clearly indicated per location. You choose the location that works best for you.

Do I need to fast before the test?

That depends on the test. Whether fasting is required, and from what time, is clearly stated in the instructions you receive after ordering. No surprises at the counter.

When and how do I receive my result?

Usually within a few working days after collection. Turnaround time varies by analysis; analyses sent to an external lab can take longer. You receive your laboratory report securely by email. It lists the measured values, units, reference ranges and any flags added by the laboratory.

Where can I go for collection?

We work with 1,000+ collection points across the Netherlands. After ordering you'll see which locations near you are available and you pick where you'd like to go.

Received a packet with everything needed, went to a walk in clinic (Amsterdam DC Clinic) where my blood was drawn and I was out the door within 5 minutes, and then after posting the samples, received results 2 days later. Results were easy to understand as well. Amazing service, would highly recommend!
