What is the Anemia Blood Test?

With anemia you have too little hemoglobin, the protein in red blood cells that carries oxygen. The Anemia Blood Test measures the blood count plus values that point to the cause, such as iron stores, vitamin B12 and kidney function.

What is tested?

  • Blood count: hemoglobin, hematocrit, red blood cells, MCV, MCH, MCHC, white blood cells and platelets
  • Reticulocytes: the production of new red blood cells
  • Iron and ferritin: the iron in your blood and your iron stores
  • Vitamin B12: needed to produce red blood cells and for the nervous system
  • ESR: a general inflammation marker
  • Creatinine and eGFR: kidney function; the kidneys produce the hormone that stimulates red blood cell production

Why should you get the Anemia Blood Test?

Anemia often develops gradually. Symptoms include fatigue, paleness, dizziness, palpitations and getting out of breath easily during exertion. Women with heavy periods, pregnant women, people who eat plant-based or donate blood often and people with a chronic illness or digestive problems are more likely to have anemia. Anemia is a signal; you determine the cause together with your GP.

What does it indicate?

The combination of values helps to identify the cause of anemia:

  • Severity: hemoglobin and hematocrit show whether there is anemia and how pronounced it is
  • Type: MCV shows whether red blood cells are small, normal or large; small cells often fit iron deficiency, large cells a B12 or folic acid deficiency
  • Production: reticulocytes show whether the bone marrow makes enough new red blood cells
  • Cause: iron, ferritin, vitamin B12, ESR and kidney function point to a deficiency, inflammation or reduced kidney function

What does a high level mean?

  • Hemoglobin and hematocrit: can be raised with dehydration, smoking or staying at altitude
  • Ferritin: can indicate iron overload, but also rises with inflammation
  • Reticulocytes: can indicate increased production, for example after blood loss or with breakdown of red blood cells
  • ESR: indicates inflammation or infection in the body

Discuss raised values with your GP.

What does a low level mean?

  • Hemoglobin: confirms anemia
  • Ferritin: is the clearest sign of iron deficiency
  • Vitamin B12: can cause anemia with large red blood cells
  • Reticulocytes: can indicate reduced production in the bone marrow, for example due to a lack of iron or B12
  • eGFR: indicates reduced kidney function, which can cause anemia

Discuss lowered values with your GP.

Preparation and sampling

  • Do not take iron tablets or a multivitamin with iron in the 24 hours before the sample is taken. They raise the value temporarily.Applies to: IJzer
  • Do you take biotin (vitamin B8, often in hair and nail supplements)? Stop at least 2 days before the sample is taken, or 4 days for a high dose.Applies to: Vitamine B12

Risks and downsides of this test

  • Blood draw at a collection point. A needle in a vein is a safe routine procedure. A bruise or slight dizziness happens; an infection of the puncture site is rare.
  • No test is one hundred percent reliable. A result can point to something that is not there, or miss something that is. A result is a snapshot and does not make a diagnosis.
  • An abnormal result has consequences. It can lead to follow-up tests with your GP, and to worry that turns out to be unnecessary. Follow-up tests or treatment can have downsides of their own.
  • You may find something other than what you were looking for. For this test that could be a blood disorder (such as leukaemia) or carrying an inherited blood disorder (such as thalassaemia). This happens occasionally. The test is not intended to find these. During checkout you choose whether you want to be informed about such an incidental finding.

Frequently asked questions

How does it work from order to result?

After ordering we generate a referral letter and email you the instructions. You pick a collection point yourself and book an appointment there. Blood is drawn at the location, processed at the lab, and your result arrives digitally.

How quickly can I get tested?

Often within a few working days. How soon you can be seen depends on how busy the location is and the type of analysis. For specialist analyses we first ship collection materials, which takes a bit longer.

Do I need to book an appointment?

At most collection points yes. Some accept walk-ins. This is clearly indicated per location. You choose the location that works best for you.

Do I need to fast before the test?

That depends on the test. Whether fasting is required, and from what time, is clearly stated in the instructions you receive after ordering. No surprises at the counter.

When and how do I receive my result?

Usually within a few working days after collection. Turnaround time varies by analysis; analyses sent to an external lab can take longer. You receive your laboratory report securely by email. It lists the measured values, units, reference ranges and any flags added by the laboratory.
