What does this test examine?

Serum cholinesterase is an enzyme mainly produced by the liver. This test measures its activity in blood.

Collect a blood sample at home with a finger-prick kit and send it to the laboratory.

Why have this test?

Testing can form part of a targeted assessment of liver protein production or suspected abnormal cholinesterase activity. The clinical question determines which further tests may be useful.

What is tested and how is the result assessed?

The result is assessed alongside other liver results, symptoms, medication and possible inherited factors. This serum test does not directly measure nervous system function.

What does a high result mean?

Increased activity can occur in several metabolic circumstances. A high value is not specific to a single condition.

What does a low result mean?

Low activity may occur with reduced liver production, inherited variants or exposure to substances that inhibit the enzyme. This home test is not intended for suspected acute poisoning.

Risks and downsides of this test

  • Finger prick at home. For about nine in ten people the sample succeeds on the first try. Too little blood or haemolysis (damaged blood cells from squeezing hard) makes the sample unusable and a new kit is needed.
  • No test is one hundred percent reliable. A result can point to something that is not there, or miss something that is. A result is a snapshot and does not make a diagnosis.
  • An abnormal result has consequences. It can lead to follow-up tests with your GP, and to worry that turns out to be unnecessary. Follow-up tests or treatment can have downsides of their own.
  • Incidental findings are not expected with this test. An incidental finding is something other than what you were looking for. During checkout you choose whether you want to be informed about one.

Liver and pancreas

Cholinesterase

An enzyme from the liver; shows how sensitive someone is to certain muscle relaxants and says something about the liver.

Everything about this test

What it measures

The test measures the activity of pseudocholinesterase (butyrylcholinesterase), an enzyme that the liver makes and releases into the blood. The enzyme breaks down certain substances, including the muscle relaxant succinylcholine that is used during general anesthesia. Because the liver makes the enzyme, its activity is also a measure of how much protein the liver can produce. In addition, some insecticides (organophosphates) inhibit the enzyme.

  • An inherited reduced activity of the enzyme, which makes certain muscle relaxants work longer during general anesthesia
  • A reduced production function of the liver
  • Poisoning with organophosphate insecticides

High

A raised activity occurs with overweight, fatty liver, a raised fat level in the blood, diabetes and with protein loss through the kidneys. A raised value causes no symptoms itself and usually has little meaning.

Low

According to the Dutch Society for Clinical Chemistry (NVKC), a low activity usually has an inherited cause: an abnormality in the structure of the enzyme. People who have this notice nothing in daily life, but can remain paralyzed much longer than expected after general anesthesia with succinylcholine. A low value can also fit severely reduced liver function, malnutrition, a serious illness or poisoning with organophosphate insecticides. Pregnancy, the contraceptive pill and some medicines slightly lower the value without there being a disease.

If your result is abnormal

With a clearly low value, it is especially important that the anesthesiologist knows this before any surgery; sometimes further testing for the inherited variant follows, and family members may then also have increased sensitivity. With a low value together with other abnormal liver results, the GP looks at the cause of the liver function disorder.

Reliability

A clearly low activity reliably points to increased sensitivity to succinylcholine; which inherited variant is behind it only becomes clear from further testing with inhibitors or DNA. As a measure of liver function, cholinesterase is less specific than albumin and the clotting time (PT or INR), because diet, pregnancy and medicines also lower the value. In people without symptoms, a slightly abnormal value says little.

Limitations

  • Pregnancy, the contraceptive pill and other estrogens lower the activity without disease.
  • Some medicines, such as certain cancer drugs, cyclophosphamide and neostigmine, lower the activity.
  • Overweight, fatty liver and a high fat level in the blood raise the value.
  • Reference values and the measurement method differ between laboratories; results from different laboratories are not easy to compare.

How common is this

An inherited reduced activity of the enzyme is not rare: about 1 in 25 to 30 people of European descent carry one abnormal variant, which at most slightly lowers the activity. A severe form, in which both genes are affected, occurs in about 1 in 3,000 to 5,000 people.

Who is this useful for

The result is mainly meaningful before general anesthesia in someone who themselves, or a family member, previously remained paralyzed for an unusually long time after surgery, when poisoning with insecticides is suspected and as an addition with known liver disease. In people without symptoms, a slightly abnormal value says little.

Downsides and risks

  • A slightly low value due to pregnancy, the pill or medicines can wrongly be read as a liver problem or inherited abnormality.

Frequently asked questions

How does the fingerstick test work from A to Z?

After ordering we ship the collection kit to your home. You prick your fingertip, collect a few drops of blood, return it in the envelope, and receive your result digitally by email.

How do I collect my blood sample?

A small prick on your fingertip lets you collect a few drops into a tube. The instructions include step-by-step photos and video, so no guessing required.

When and how do I receive my result?

Usually 2 to 5 working days after the lab receives your sample. You receive your laboratory report securely by email. It lists the measured values, units, reference ranges and any flags added by the laboratory.

What if the collection doesn't work?

Some people find the fingerstick tricky at first. Warm your hands, hang your arm down, and prick the side of the fingertip for better blood flow. If it still doesn't work, get in touch.

How do I return the collection materials to the lab?

Use the supplied return envelope and place it in the letter-mail bag inside a PostNL service point yourself as soon as possible. Do not hand it to counter staff or request track & trace. The instructions supplied with your test state when it must be sent.

What if I have questions or something doesn't work?

Feel free to contact us via WhatsApp, email or phone. We're happy to help you before, during or after your test. If the collection doesn't work, we'll arrange a solution or send new materials — no hassle.

“Smooth process and good support”

Amazing website experience. I ordered various tests for 2 people. We went to one of the supported centers and took samples. Also after the results were ready, I needed a letter from the laboratory and they prepared the letter for me in no time. Definitely recommended.

“Amazing service!”

Received a packet with everything needed, went to a walk in clinic (Amsterdam DC Clinic) where my blood was drawn and I was out the door within 5 minutes, and then after posting the samples, received results 2 days later. Results were easy to understand as well. Amazing service, would highly recommend!
