What does this test examine?

Laboratory PCR for Mycoplasma hominis.

What is analysed?

  • Mycoplasma hominis (PCR)

Choose the appropriate test option

The analyses are the same. The collection method and supplied materials differ by test option.

  • Men – urine
  • Women – vaginal swab

When can you test?

An STI may not yet be detectable shortly after exposure. The appropriate testing time differs by infection. If you have symptoms, are pregnant, use PrEP or PEP, had a very recent exposure or experienced sexual violence, contact your doctor or sexual health clinic.

Your result

You receive the laboratory result digitally. A positive or reactive result may require additional testing or treatment through a doctor or sexual health clinic.

Preparation and sampling

  • Collect the urine when you have not passed urine for at least 1 hour.
  • Collect only the first part of the stream, not the middle part.

Risks and downsides of this test

  • Collecting urine. The quality of the sample depends on when and how it is collected. A wrongly collected sample can distort the result.
  • No test is one hundred percent reliable. A result can point to something that is not there, or miss something that is. A result is a snapshot and does not make a diagnosis.
  • An abnormal result has consequences. It can lead to follow-up tests with your GP, and to worry that turns out to be unnecessary. Follow-up tests or treatment can have downsides of their own.
  • Incidental findings are not expected with this test. An incidental finding is something other than what you were looking for. During checkout you choose whether you want to be informed about one.

STIs

Mycoplasma hominis PCR (urine)

PCR test for Mycoplasma hominis in urine; often present without symptoms, so a positive result says little.

Everything about this test

What it measures

Mycoplasma hominis is a small bacterium without a cell wall that is found in the genital organs of many healthy people without causing symptoms (carriership). The bacterium can be transmitted through sex, but is not counted as an STI, because it has not been shown to cause disease in adults. In women, it is mainly found in larger amounts with bacterial vaginosis (a disturbance of the normal bacteria in the vagina). The PCR looks for genetic material of the bacterium in the first part of the urine and does not say whether the bacterium causes symptoms.

  • Presence of Mycoplasma hominis in the urethra; this is usually carriership without symptoms

Positive

A positive result means that the bacterium is present. This is common in people without symptoms and usually does not mean disease. According to a position statement of the European STI guidelines editorial board (IUSTI, 2018), there is no good evidence that Mycoplasma hominis causes inflammation or infertility in adult men or non-pregnant women, and it has not been shown that treatment does more good than harm. Treating a positive result without symptoms is therefore not needed. In men, according to that recommendation, a positive result often reflects the bacteria of a female partner.

Negative

A negative result means that the bacterium was not found. Because Mycoplasma hominis also occurs in healthy people and is not a proven pathogen, a negative result has little meaning for your health. This result neither explains nor rules out urethral symptoms; tests for, among others, chlamydia, gonorrhea and Mycoplasma genitalium are intended for that.

Preparation

  • Collect the urine when you have not passed urine for at least 1 hour.
  • Collect only the first part of the stream, not the middle part.

If your result is abnormal

With a positive result without symptoms, no treatment or follow-up check is needed according to the European recommendation. With symptoms, the GP first looks for other causes, such as chlamydia, gonorrhea, trichomonas or, in women, bacterial vaginosis. Mycoplasma hominis is not notifiable.

Reliability

The PCR detects the bacterium reliably, but a positive result does not distinguish between carriership and an infection that causes symptoms. Mycoplasma hominis is not a proven pathogen: the evidence that this bacterium causes urethritis or other symptoms is insufficient. As a result, a positive result says little, both in people without symptoms and in people with symptoms.

Limitations

  • A positive result does not say whether the bacterium causes symptoms; many people without symptoms also carry the bacterium.
  • In men, according to the European recommendation, the bacterium is not linked to urethritis.
  • Mycoplasma hominis is naturally insensitive to some of the common antibiotics and often does not disappear permanently after treatment.
  • If you urinated less than 1 hour before collecting the sample, or collect more than the first part of the stream, the sample is diluted and the test can miss an infection.

How common is this

How often Mycoplasma hominis occurs in the Netherlands is not known, because it is not routinely tested for. In international research using PCR, the bacterium was found in 3 to 15 percent of sexually active women who were not pregnant. In men who visited an STI clinic, this was 2 to 4 percent; most people with the bacterium do not become ill.

Who is this useful for

The result usually does not explain symptoms, because the bacterium also occurs in many healthy people. Urethral symptoms more often fit chlamydia or gonorrhea, and, with symptoms lasting longer than 4 weeks, Mycoplasma genitalium. This test is not suitable for questions about pregnancy or fertility.

Downsides and risks

  • A positive result can lead to unnecessary treatment with antibiotics, with a risk of side effects and of resistance in this bacterium and in other bacteria, including the causes of real STIs.
  • A positive result can wrongly give the idea that you have an STI, resulting in worry or tension in a relationship.

Frequently asked questions

View the collection instructionsFull instructions with visual guidance
How does the STI urine test work from order to result?

You receive the collection kit at home, collect the first part of the urine stream, fill the transport tube following the included instructions and return the sample to Medlab Stein. You receive the laboratory result digitally.

Which part of the urine stream should I collect?

For this STI PCR, collect the first part of the urine stream, not a midstream sample. The exact waiting time beforehand and required volume are stated in the kit instructions; always follow those values.

Why is this different from a regular urine test?

An STI PCR examines the urogenital sample for genetic material from pathogens. First-catch urine is the intended specimen type, so general midstream urine instructions do not apply here.

Does urine also test for an infection in my throat or rectum?

No. The result only applies to the urogenital body site. After oral or anal exposure, you need a separate throat or rectal swab for that site.

How do I return the collection materials to the lab?

Use the supplied return envelope and place it in the letter-mail bag inside a PostNL service point yourself as soon as possible. Do not hand it to counter staff or request track & trace. The instructions supplied with your test state when it must be sent.

What if I have questions or something doesn't work?

Feel free to contact us via WhatsApp, email or phone. We're happy to help you before, during or after your test. If the collection doesn't work, we'll arrange a solution or send new materials — no hassle.

Amazing website experience. I ordered various tests for 2 people. We went to one of the supported centers and took samples. Also after the results were ready, I needed a letter from the laboratory and they prepared the letter for me in no time. Definitely recommended.
