What is the Testosterone Replacement Therapy (TRT) Blood Test?

The TRT Blood Test is a comprehensive blood analysis specifically developed to check if you could benefit from testosterone replacement therapy. This test measures not only your testosterone levels, but also important hormones that regulate testosterone production, as well as markers for liver, blood, and prostate health. It is essential for men with symptoms of low testosterone such as fatigue, decreased libido, mood swings, and loss of muscle mass. If suitable, a physician can directly prescribe and guide TRT.

What is tested?

  • Testosterone (total) – The primary male sex hormone
  • LH (Luteinizing hormone) – Hormone that stimulates testosterone production in the testes
  • FSH (Follicle-stimulating hormone) – Regulates sperm production
  • Estradiol – Main estrogen, converted from testosterone
  • Prolactin – Hormone that can inhibit testosterone production at elevated levels
  • PSA (Prostate-specific Antigen) – Marker for prostate health
  • Hemoglobin – Oxygen-transporting protein in red blood cells
  • Hematocrit (Ht) – Percentage of red blood cells in blood
  • ALT (Alanine transaminase) – Liver enzyme
  • AST (Aspartate transaminase) – Liver enzyme
  • Gamma-GT – Liver enzyme for bile duct function
  • Alkaline phosphatase – Enzyme for liver and bone health

Why should you get the TRT Blood Test?

This test is crucial for men considering starting TRT or those already using TRT and wishing to monitor their levels. Symptoms of low testosterone such as chronic fatigue, decreased libido, erectile dysfunction, loss of muscle mass, increase in body fat, and depressive feelings can significantly reduce quality of life. The test provides a complete picture of your hormonal status and checks important safety markers such as liver function, blood counts, and prostate health that are important for TRT treatment.

What does it indicate?

The results provide comprehensive insight into your hormonal health and suitability for TRT. This test measures:

  • Testosterone levels and regulation – Low testosterone with low LH/FSH indicates secondary hypogonadism (brain), low testosterone with high LH/FSH indicates primary hypogonadism (testes)
  • Estradiol – Elevated levels can cause side effects
  • Prostate health – PSA levels are essential because TRT can affect the prostate
  • Liver and blood values – Important for safe treatment

This information helps physicians develop a personalized TRT plan with optimal dosage and monitoring.

What does a high level mean?

  • High testosterone levels during TRT – May indicate an excessively high dosage
  • Elevated hematocrit – A known side effect of TRT that increases the risk of blood clots
  • Elevated liver enzymes – May indicate liver strain
  • High PSA value – Requires further investigation for prostate problems
  • Elevated estradiol – Can lead to fluid retention, gynecomastia, and mood swings

These values help to safely optimize treatment.

What does a low level mean?

  • Low testosterone levels – Confirm the diagnosis of hypogonadism and justify TRT treatment
  • Low LH/FSH with low testosterone – Indicate a problem in the pituitary or hypothalamus
  • Too low hematocrit during TRT – May indicate anemia
  • Low estradiol – Can lead to joint problems and decreased bone density

This information helps physicians develop a personalized TRT plan with optimal dosage and monitoring.

Frequently asked questions

Answers to what people usually want to know.

How does it work from order to result?

After ordering we generate a referral letter and email you the instructions. You pick a collection point yourself and book an appointment there. Blood is drawn at the location, processed at the lab, and your result arrives digitally.

How quickly can I get tested?

Usually very quickly. Collection points typically have slots within a few days, sometimes even same-day. It depends on how busy the location is. For a few specialist analyses we first ship collection materials, which takes a bit longer.

Do I need to book an appointment?

At most collection points yes. Some accept walk-ins. This is clearly indicated per location. You choose the location that works best for you.

Do I need to fast before the test?

That depends on the test. Whether fasting is required, and from what time, is clearly stated in the instructions you receive after ordering. No surprises at the counter.

When and how do I receive my result?

It varies per analysis: from same-day to a few working days. Standard values often come back quickly; analyses that go to an external lab can take a bit longer. You get an email with a link to your digital result, including a short explanation per value.

Where can I go for collection?

We work with 1,000+ collection points across the Netherlands. After ordering you'll see which locations near you are available and you pick where you'd like to go.
