What does this test examine?

Immunoglobulin A (IgA) is a class of antibody. IgA is found in blood and also contributes to immune protection at mucosal surfaces, including the airways and intestines. This test measures total IgA in blood.

For this test, have a blood sample taken at a collection location.

Why have this test?

IgA testing can form part of an investigation into recurrent infections or a possible antibody deficiency. Total IgA can also help with interpreting certain coeliac disease tests.

What is tested and how is the result assessed?

The result shows the total IgA concentration. It does not measure antibodies against a particular infection or food. Interpretation takes account of symptoms and, where relevant, other blood results.

What does a high result mean?

Raised IgA may occur with inflammation, liver disease or increased antibody production. A single elevated result does not establish a diagnosis.

What does a low result mean?

Low IgA may reflect IgA deficiency or another problem with antibody production. Some people with low IgA have few symptoms; discuss the result with your doctor.

Risks and downsides of this test

  • Blood draw at a collection point. A needle in a vein is a safe routine procedure. A bruise or slight dizziness happens; an infection of the puncture site is rare.
  • No test is one hundred percent reliable. A result can point to something that is not there, or miss something that is. A result is a snapshot and does not make a diagnosis.
  • An abnormal result has consequences. It can lead to follow-up tests with your GP, and to worry that turns out to be unnecessary. Follow-up tests or treatment can have downsides of their own.
  • You may find something other than what you were looking for. For this test that could be an abnormal protein in the blood (M-protein, as in MGUS). This is rare. The test is not intended to find these. During checkout you choose whether you want to be informed about such an incidental finding.

Inflammation and immunity

IgA

The amount of IgA antibodies; shows an IgA deficiency, which also matters for the celiac disease test.

Everything about this test

What it measures

Immunoglobulins are defense proteins (antibodies) that white blood cells make to clear bacteria, viruses and other invaders. IgA is the type that mainly works on the mucous membranes: in the nose and throat, airways, intestines, saliva and breast milk. The test measures the total amount of IgA in the blood, so not the defense against one particular pathogen. In young children, the value is lower than in adults.

  • IgA deficiency (too little IgA), often without symptoms
  • A false-negative celiac disease test: the standard test uses IgA antibodies, which are missing with an IgA deficiency
  • A broader immune disorder, if IgG and IgM are also low
  • Rarely: a condition in which one group of immune cells makes too much of one type of immunoglobulin (M protein)

High

A slightly raised IgA is common during or shortly after an infection and is then usually harmless. Long-lasting inflammation, autoimmune diseases and liver diseases can also increase the amount of immunoglobulins. The raised value itself does not cause symptoms; any symptoms belong to the cause. Rarely, a strongly raised value fits a condition in which one group of immune cells makes too much of one type of immunoglobulin (an M protein); this is investigated further with protein electrophoresis.

Low

A low or undetectable IgA (IgA deficiency) often causes no symptoms and is sometimes discovered by chance, for example when registering as a blood donor. Other people, on the other hand, have more frequent infections of the sinuses, ears, lungs, intestines or urinary tract. With an IgA deficiency, the usual celiac disease test can wrongly come out negative, because it relies on IgA antibodies. In children under 4, a low value can be normal, because the body does not yet make IgA well at that age. Some people with IgA deficiency can become seriously ill from blood or blood products; it is important for doctors to know this.

If your result is abnormal

With a low IgA, the GP usually also measures IgG and IgM and, when celiac disease is in question, IgG antibodies against transglutaminase. With repeated or severe infections, a referral follows to an internist (specialist in internal medicine), immunologist or pediatrician. A strongly raised value without a clear cause is usually investigated further with protein electrophoresis.

Reliability

IgA can be measured accurately, but a single value says little about how well your immune system works. According to the Dutch Society for Clinical Chemistry (NVKC), total IgA is mainly measured alongside the celiac disease test, to rule out a false-negative result due to IgA deficiency; IgA deficiency is defined as a value below 0.07 g/l. With frequently recurring infections, IgA is assessed together with IgG and IgM. The cause of a value that is too high only becomes clear with further tests.

Limitations

  • Reference values depend on age and differ by laboratory.
  • A recent or ongoing infection or inflammation can temporarily raise the value.
  • Some medicines can reduce the production of IgA.
  • The result says nothing about the defense against a specific pathogen.

How common is this

About 1 in 700 people has an IgA deficiency, usually without symptoms. IgA deficiency is more common in people with celiac disease. How often a slightly raised IgA due to an infection or inflammation is found has not been recorded in Dutch figures.

Who is this useful for

Useful with frequently recurring infections of the airways, ears, sinuses or intestines, and as an addition to testing for celiac disease to rule out a false-negative result. In young children, a low value is often normal. For people without symptoms, the test yields little: an IgA deficiency found by chance usually causes no symptoms and is not treated.

Downsides and risks

  • A low or slightly raised IgA found by chance without symptoms can cause unnecessary worry and follow-up tests.
  • A normal value does not mean that the immune system works well against all infections.

Frequently asked questions

How does it work from order to result?

After ordering we generate a referral letter and email you the instructions. You pick a collection point yourself and book an appointment there. Blood is drawn at the location, processed at the lab, and your result arrives digitally.

How quickly can I get tested?

Often within a few working days. How soon you can be seen depends on how busy the location is and the type of analysis. For specialist analyses we first ship collection materials, which takes a bit longer.

Do I need to book an appointment?

At most collection points yes. Some accept walk-ins. This is clearly indicated per location. You choose the location that works best for you.

Do I need to fast before the test?

That depends on the test. Whether fasting is required, and from what time, is clearly stated in the instructions you receive after ordering. No surprises at the counter.

When and how do I receive my result?

Usually within a few working days after collection. Turnaround time varies by analysis; analyses sent to an external lab can take longer. You receive your laboratory report securely by email. It lists the measured values, units, reference ranges and any flags added by the laboratory.

Where can I go for collection?

We work with 1,000+ collection points across the Netherlands. After ordering you'll see which locations near you are available and you pick where you'd like to go.

“Amazing service!”

Received a packet with everything needed, went to a walk in clinic (Amsterdam DC Clinic) where my blood was drawn and I was out the door within 5 minutes, and then after posting the samples, received results 2 days later. Results were easy to understand as well. Amazing service, would highly recommend!
