What does this test examine?

Anti-Tg are antibodies against thyroglobulin, a protein in the thyroid gland. This test measures the antibodies rather than thyroglobulin itself.

Collect a blood sample at home with a finger-prick kit and send it to the laboratory.

Why have this test?

Testing can help investigate an autoimmune response against the thyroid. Anti-Tg is also relevant when interpreting thyroglobulin results because these antibodies may interfere with the measurement.

What is tested and how is the result assessed?

The result is assessed alongside symptoms and other thyroid tests, such as TSH, free T4 and, where relevant, anti-TPO. Antibodies alone do not show whether thyroid function is overactive or underactive.

What does a high result mean?

A raised result can occur with autoimmune thyroid conditions, including Hashimoto disease and Graves disease. The result alone does not establish a diagnosis.

What does a low result mean?

A low or undetectable result means little or no anti-Tg was found with this method. It does not exclude every thyroid condition.

Preparation and sampling

  • Do you take biotin (vitamin B8, often in hair and nail supplements)? Stop at least 2 days before the sample is taken, or 4 days for a high dose.
More about fasting and preparation

Risks and downsides of this test

  • Finger prick at home. For about nine in ten people the sample succeeds on the first try. Too little blood or haemolysis (damaged blood cells from squeezing hard) makes the sample unusable and a new kit is needed.
  • No test is one hundred percent reliable. A result can point to something that is not there, or miss something that is. A result is a snapshot and does not make a diagnosis.
  • An abnormal result has consequences. It can lead to follow-up tests with your GP, and to worry that turns out to be unnecessary. Follow-up tests or treatment can have downsides of their own.
  • Incidental findings are not expected with this test. An incidental finding is something other than what you were looking for. During checkout you choose whether you want to be informed about one.

Thyroid

Anti-Tg

Antibodies against thyroglobulin, a protein from the thyroid; found in autoimmune diseases of the thyroid.

Everything about this test

What it measures

Thyroglobulin (Tg) is a protein that is made only in the thyroid and in which thyroid hormone is built. In an autoimmune reaction against the thyroid, the immune system can make antibodies against this protein: anti-Tg. The test measures how many of these antibodies are in the blood. In the hospital, anti-Tg is mainly measured alongside thyroglobulin, for monitoring after removal of the thyroid, because the antibodies can interfere with that measurement.

  • A sign of an autoimmune disease of the thyroid, such as Hashimoto's disease
  • Interference with the thyroglobulin measurement by antibodies (hospital setting)

High

A raised anti-Tg occurs in various thyroid diseases, especially in autoimmune diseases such as Hashimoto's disease and Graves' disease. People without thyroid disease can also have raised anti-Tg, without this having consequences. A raised value says nothing about how well the thyroid is working now; TSH and free T4 show that. With normal thyroid function, a raised anti-Tg on its own usually means nothing for treatment.

Low

Most people have no anti-Tg; a low or undetectable value is normal. This does not rule out an autoimmune disease of the thyroid: in Hashimoto's disease, anti-TPO is found more often than anti-Tg.

Preparation

  • Do you take biotin (vitamin B8, often in hair and nail supplements)? Stop at least 2 days before the sample is taken, or 4 days for a high dose.

If your result is abnormal

With a raised anti-Tg, the GP looks at TSH and free T4. If these are normal, there is usually no further testing; with symptoms that point to a thyroid disorder, TSH can be measured again later.

Reliability

Anti-Tg says nothing about how well the thyroid is working; thyroid function is determined with TSH and free T4, according to the Dutch GP guideline (NHG) on thyroid disorders. According to the Dutch Society for Clinical Chemistry (NVKC), the test is mainly intended to assess whether a thyroglobulin measurement in the hospital is reliable. As a sign of an autoimmune disease of the thyroid, anti-Tg adds little alongside anti-TPO.

Limitations

  • People without thyroid disease can also have raised anti-Tg.
  • Reference values and cut-off values differ per laboratory and measurement method; only compare results from the same laboratory.
  • The result says nothing about current thyroid function; without TSH alongside it, it is hard to interpret.

How common is this

An underactive thyroid affects about 3 to 4 in 100 adults in the Netherlands and an overactive thyroid about 1 in 100, both more often women. How often anti-Tg occurs in people without symptoms is not well known.

Who is this useful for

The result is most meaningful in the hospital, alongside a thyroglobulin measurement after removal of the thyroid. With thyroid symptoms, TSH shows how the thyroid is working; anti-Tg says nothing about that. As a sign of an autoimmune disease of the thyroid, TSH and anti-TPO usually say more than anti-Tg.

Downsides and risks

  • A raised anti-Tg with normal thyroid function can lead to unnecessary worry or unnecessary follow-up testing.

Frequently asked questions

See fingerstick instructionsFull instructions with visual guidance
How does the fingerstick test work from A to Z?

After ordering we ship the collection kit to your home. You prick your fingertip, collect a few drops of blood, return it in the envelope, and receive your result digitally by email.

How do I collect my blood sample?

A small prick on your fingertip lets you collect a few drops into a tube. The instructions include step-by-step photos and video, so no guessing required.

When and how do I receive my result?

Usually 2 to 5 working days after the lab receives your sample. You receive your laboratory report securely by email. It lists the measured values, units, reference ranges and any flags added by the laboratory.

What if the collection doesn't work?

Some people find the fingerstick tricky at first. Warm your hands, hang your arm down, and prick the side of the fingertip for better blood flow. If it still doesn't work, get in touch.

How do I return the collection materials to the lab?

Use the supplied return envelope and place it in the letter-mail bag inside a PostNL service point yourself as soon as possible. Do not hand it to counter staff or request track & trace. The instructions supplied with your test state when it must be sent.

What if I have questions or something doesn't work?

Feel free to contact us via WhatsApp, email or phone. We're happy to help you before, during or after your test. If the collection doesn't work, we'll arrange a solution or send new materials — no hassle.

Received a packet with everything needed, went to a walk in clinic (Amsterdam DC Clinic) where my blood was drawn and I was out the door within 5 minutes, and then after posting the samples, received results 2 days later. Results were easy to understand as well. Amazing service, would highly recommend!
