Zonuline is een proteïne, dat actief is in het darmepitheel. Zonuline reageert op blootstelling aan gliadine en pathogene bacteriën. Gliadine vormt samen met glutenine het eiwit gluten.
Zodra zonuline wordt vrijgemaakt door de epitheelcellen, wordt dit gesignaleerd door Immunoglobuline A, waarna een reactie volgt van de darmwand. De verbindingseiwitten (tight junctions) tussen de cellen van de darmwand verslappen of verbreken. De gliadinen of pathogene bacteriën kunnen nu de darmwand passeren, waarna dendritische cellen via de antigeen-presenterende cellen, een T-celactivatie uitlokken.
Deze immuunrespons veroorzaakt inflammatie van de cellen van de darmwand, waarna zonuline, tight junction-eiwitten, pathogenen, bacteriële lipopolysaccharides en ontstekingsmediërende cytokinen vanuit de darmen het lamina propria kunnen binnendringen. Vervolgens komen deze stoffen in de bloedbaan, waardoor ze neuroinflammatie kunnen uitlokken.
