Zonulin is a protein that is active in the intestinal epithelium. Zonulin reacts to exposure to gliadin and pathogenic bacteria. Gliadin, together with glutenin, forms the protein gluten.
As soon as zonulin is released by the epithelial cells, it is signaled by Immunoglobulin A, after which a reaction of the intestinal wall follows. The tight junction proteins between the cells of the intestinal wall loosen or break. The gliadins or pathogenic bacteria can now pass through the intestinal wall, after which dendritic cells, via the antigen-presenting cells, trigger T-cell activation.
This immune response causes inflammation of the cells of the intestinal wall, after which zonulin, tight junction proteins, pathogens, bacterial lipopolysaccharides, and inflammatory mediating cytokines can enter the lamina propria from the intestines. Subsequently, these substances enter the bloodstream, where they can induce neuroinflammation.
The underlying condition or the clinical significance of this test is insufficiently supported within mainstream Dutch medicine.
Rationale for this test
De zonuline-test in ontlasting wordt binnen de Nederlandse reguliere geneeskunde niet onderbouwd. Het onderliggende concept 'leaky gut syndroom' is geen erkend ziektebeeld in de reguliere zorg. De wetenschappelijke basis voor zonuline-meting als diagnosticum voor darmwandintegriteit is onvoldoende, en de test is niet opgenomen in NHG- of FMS-richtlijnen.
Do not interpret a result from this test as a diagnosis, and always discuss with your GP which regular diagnostics are appropriate for your symptoms.

Lay plastic film over the toilet or use a clean container. Scoop from the middle of the stool and avoid contact with urine or toilet water.
A small scoop with the included spoon is enough — about half a teaspoon. More is not better.
Maximum 24 to 48 hours in the fridge. Send as soon as possible and preferably early in the week, not just before the weekend.
Some analyses (like calprotectin) ask you to avoid NSAIDs or certain supplements 24-48 hours before collection. Check the instructions with your kit for the exact preparation for your test.
Stool analyses typically take about a week after the lab receives your sample. You get an email with a link to your digital result, including explanation per value.
